Access & Referral Information

When looking for mental health services, you want the security of knowing you can access the care you need. The Access to Care department connects you with adequate support until services are initiated.
 
When calling the Access department, you will speak with one customer service provider. That person is your single point of contact until you meet with a qualified mental health professional. If you have questions about offered services, specialized clinicians, covered insurances, need crisis intervention or would like to start any type of offered service with Horizons, the Access to Care Team can help. 
 
Trained, knowledgeable, friendly, helpful and courteous Access to Care team staff are available during regular business hours. We are here to make your entrance into our agency as smooth and seamless as possible. We look forward to working with and serving you.
 
What will I need for my first appointment?
  • Either the  pediatric or adult health history form
  • Proof of income to apply for our sliding scale fee program
  • All current medications
  • Insurance or Medical Card
  • Social security number
